Montachusett Veterans Outreach Center offers a variety of housing and housing retention options for Veterans, which you can read more about here. To ensure your are placed in the program that is the best fit, and also with the correct case manager, there is both an application packet and a clinical assessment that needs to be completed.
The application packet consists of:
- Intake packet (download and open with Adobe to sign electronically)
- VA Request for and Authorization to release health information
- VA 1010-EZ form (for veterans without VA healthcare)
- VA 101-EZR form (for Veterans already in the VA healthcare system)
Both the intake packet and VA Release form must completed in full prior to moving on to the clinical assessment. For GPD applicants, the VA 1010-EZ form must also be completed prior to moving on to the clinical assessment.
