Comprehensive Case Management
MVOC case managers are vital in our transitional and permanent-supportive residential reintegration programs. Compassionate and fair, they provide personal coaching and mentoring, and facilitate resources tailored to each client-resident.

Complete the Intake Application
All documents Completed
The required documents include the VA Request for and Authorization to Release Health Information and either the VA 1010-EZ Form (for veterans without VA healthcare) or the VA 1010-EZR Form (for veterans already enrolled in the VA healthcare system). Both the VA 1010 form and VA Release form must be fully completed & submitted before the clinical assessment.
Undergo Clinical Assessment
Participate in a review of your veteran status, health, and housing history to determine the best program fit for your needs.
Apply for MVOC Housing & Case Management


To ensure correct placement, veterans must complete an intake application and undergo a review of their veteran status, health, and housing history. Our application packet and subsequent clinical intake ensure you are placed in the housing and/or case management program that is the right fit for your needs. The application packet includes:
- Intake packet (download and open with Adobe to sign electronically)
- VA Request for and Authorization to release health information
- VA 1010-EZ form (for veterans without VA healthcare)
- VA 101-EZR form (for Veterans already in the VA healthcare system)
Both the intake packet and VA Release form must be fully completed before the clinical assessment. GPD applicants must also complete the VA 1010-EZ form before the assessment.
